Objectives

After participating in this educational activity, attendees should be able to:
1. Describe local efforts to create a Rural Health Network in Franklin County and the North Quabbin, pooling resources and creating a 'no wrong door' model to support the needs of the population.
2. Name at least three community-based service providers also supporting the health of our patient population.
3. Express increased confidence in our ability to identify, engage, assess, service and manage the population in our service area..
