Objectives

After participating in this educational activity, attendees should be able to:
1. Describe the roles of the members of the healthcare team in their respective responsibilities for quality care.
2. Describe the rationale for why collaboration with community partners and coordination across the continuum of care are essential components in a Population Health  model of care in Franklin County and the N. Quabbin and why attention to opioid use disorder is a priority.
3. Explain why confidence in the assessment and management of acute and chronic pain and communication of an
evidence-based approach with individuals suffering with pain is critical to the health and well-being of the population.
4. Describe new JCAHO guidelines for the assessment and management of pain.
5. Explain how we can best serve the Population through the integration of medicine, behavioral health and social services across the continuum of care, while improving the experience of care for the individual and work satisfaction for members of their care teams.
6. Explain how trauma and the assessment determined using the Adverse Childhood Experiences Score (ACES) impacts health outcomes, including the propensity for addiction.
7. Describe why Medication Assisted Treatment (MAT) can be an effective intervention as part of recovery.
8. Describe how best to identify, engage, assess, serve and manage the care of individuals suffering with opioid addiction; best practices, lessons learned, suggestions for timelines and next steps.
9. Determine level of commitment to a brain trust of stakeholders across the continuum of care that are willing to share assets, pool resources and participate in developing the next steps in improving experience and continuity of care for individuals of our community suffering with the chronic disease of opioid use disorder..
