Goal
The goal for this session is to participate in open discussion and transparent culture for healthcare quality and patient safety

Objectives

After participating in this educational activity, attendees should be able to:
1. Evaluate adverse events or near misses thoroughly and honestly.
2. Analyze root causes  that may have contributed to the failures
3. Identify approaches to address the existing problems to reduce risk and improve patinet safety
4. Formulate a follow-up plan to affect changes in physician behaviors, systems, or processes
5. Develop and improve processes/workflow and reduce errors or the potential for errors in pathology
