This group started back in 2009 in collaboration with IHI, Institute of Healthcare Improvement, as STAAR (State Action on Avoidable Re-hospitalizations) Initiative, and ended in 2013. This group was a groundbreaking, multi-state, multi-stakeholder approach to improve the delivery of care at a regional scale. The Aim was to reduce re-hospitalizations by working across organizational boundaries and by engaging payers, stakeholder at the state, regional, and national level, patients and families, and caregivers at multiple sites and clinical interfaces. After the 4 year collaborative, this group knew the work was critical and agreed to continue the collaboration under the name of Western Mass Transitions of Care Cross Continuum (WM TICCC). Topics of focus are around populations of patients; and broken processes, to prevent avoidable readmissions. The group determines the subject matter, according to the needs of its members. Best practices are researched, shared, and worked on together.
