Twice monthly outpatient clinicians caring for older adults meet for an hour in a virtual conference.  Attendees present challenging clinical cases for input from other participants and the core team of inter-professional experts in in geriatrics.  The sessions conclude with a brief didactic on a geriatrics topic.  The case presentations and curriculum focus on the "4 Ms" of age friendly care (what matters, medications, mentation and mobility.)  Participants also receive coaching on quality improvement tools and principles and work on a small project to improve care for older adults.  The clinicians are able to earn certificates of accomplishment in geriatrics professional development.

In support of improving patient care, Baystate Health is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
