Program Goal
The goal of this educational activity is to educate providers on how to determine decision-making capacity (especially in regards to appointing HCP) and how to complete Guardianship Forms (MPC 400 and MPC 800) when needed.
Target Audience
This educational activity is designed for Physicians, Residents, Fellows, APP’s and Medical Students though primary audience is Hospitalists
Learner Objectives
After participating in this educational activity, attendees should be able to:
- Determine capacity to appoint Health Care Proxy in many cases.
- Understand pitfalls of determination in patients with aphasia or cognitive impairment.
- Complete the MPC 400 (Guardianship) form for most med-surg patients who need it.
- Complete the MPC 800 ("Rogers) form for most med-surg patients who need it.
