Objectives

After participating in this educational activity, attendees should be able to:

1| State at least 3 importance tasks of good case management to meet individual patient needs, keep patients safe at home, and decrease hospitalization risk. 
2| Discuss the importance of ongoing communication between the team and that all communication must be documented in the EMR. 
3| Recognize what they need to keep track of, what needs updating within the chart, be able to schedule or triage their caseload, and understand time management.
