Objectives

After participating in this educational activity, attendees should be able to:

1| State at least 3 interventions to help prevent patient falls, keep patients safe at home, and decrease risk for rehospitalization. 
2| Discuss the importance of on-going education and utilization of individualized care plans to help prevent falls and reduce the risk of injury.
3| Discuss how to report a fall and implement a fall prevention program to reduce reoccurrence.
