"This session of Schwartz Rounds will review the current structure of Department of Developmental Services (DDS) Shared Living and Group home residential care models.  Barriers to effective communication between caretakers who know the patients very well are often present when patients are admitted to the hospital, sometimes dramatically and adversely affecting the management plan, the care rendered and patient outcomes. We'll raise awareness of these issues and discuss strategies to improve communication among various providers, caretakers and team members to help the overall patient experience and achieve better outcomes." 

Objectives

After participating in this educational activity, attendees should be able to:
1|Explain the structure of the current Department and Developmental Service Residential Care Models, including Shared Living and Group Homes.
2|Demonstrate examples of challenges in communication between outpatient caretakers and hospital staff when these residents are admitted to the hospital.
3|Raise awareness of the barriers that exist that interfere with the best patient care and the best patient experience.
4|Suggest and discuss strategies to help improve communication among providers and with the patients to overcome barriers and improve the overall care and the patient experience.
