Objectives

After participating in this educational activity, attendees should be able to:

1| Describe key barriers contributing to delayed transitions of care for patients aged 21 and over at Baystate General Pediatric Clinic, using quality improvement tools to analyze contributing factors. (MA)

2| Identify targeted strategies to reduce the number of patients over 21 who remain empaneled to Baystate General Pediatric, applying a systematic quality improvement approach to guide intervention design and implementation (MA)

3| Discuss the need/desire for newborn education classes from families in the communities surrounding Baystate General Pediatrics, Springfield – High Street Clinic (AK)

4| Discuss the newborn education research project, barriers faced during the project and possible ways to overcome these barriers

5| Identify barriers to obtaining pre-participation physicals and their impact on youth sports (DT)

6| Evaluate potential strategies to improve access to sports physicals for student athletes (DT)
