Objectives

After participating in this educational activity, attendees should be able to:

1| Identify and document how we can show improvement on patient's that still require supervision functionally at discharge.
2| Describe how to score the discharge Oasis based on CMS guidance.  
3| Describe the look-back process to compare Oasis scores from SOC to present day.
