Objectives

After participating in this educational activity, attendees should be able to:
1. Recognize and discuss the challenges in caring for older adults with multiple medical issues and the ways in which priorities-aligned decision making can simplify care.
2. Discuss tools to elicit what matters most to patients, including individual patient values, specific and meaningful health outcome goals, and healthcare preferences.
3. Apply strategies to align health care decision making with individual patient priorities.
