Objectives

After participating in this educational activity, attendees should be able to:
1|Review the importance of good case management to meet individual patient needs, keep patients safe at home, and decrease hospitalization risk.  
2|Discuss the utmost importance of ongoing communication between the team and the need for all communication to be documented in the EMR.  
3|Recognize what they need to keep track of, what needs updating within the chart, be able to schedule or triage their caseload, and understand time management.
