Objectives

After participating in this educational activity, attendees should be able to:
1Review the importance of good case management to meet individual patient needs, to keep patient safe at home, and decrease hospitalization risk.  
2|Staff will be able to acknowledge that on-going communication between the team is of utmost importance and that all communication must be documented in the EMR.  
3|Staff will be able to recognize what they need to keep track of, what needs updating within the chart, be able to schedule or triage their case load, as well as understanding time management.
