Description:
"The CARe program can be described briefly as a framework to facilitate communication between patients and providers when they experience an unexpected outcome. The main goal is to ensure ongoing communication in support of the patient whether that involves changes in a treatment plan, or the findings of an investigation, or both. The framework is intended to promote a culture of safety, transparency between providers and their patients, and ensure that both patients and providers receive the support they need during the process, which includes peer support among other resources to support our providers."
