Description: This session will discuss the experience of a young patient who recently died on Inpatient Hospice at Baystate after a long and arduous battle with Systemic Lupus Erythematous. Her journey from initial outpatient diagnosis through hospital dependence is one that touched the lives of many healthcare providers and staff at Baystate. Members of her care team from Primary Care, Renal, Palliative Care and Hospice will reflect on their time working with her, and then we will open the floor for reflections from the larger care community

Objectives

After participating in this educational activity, attendees should be able to:

1Discuss the unique stressors of caring for a young adult who is in the process of serious illness management.  
2|Reflect on the decision to start and then ultimately stop dialysis for a young person. 
3|Explore our struggles to support medically complex patients who also experience transportation, housing and financial insecurity.
4|Delve into how social isolation and trauma impact processes inherent to end-of-life.  
5|Examine the unique grief that healthcare providers may experience at the passing of a well-known patient.
