Objectives.

  1. Understand the current state of evidence on different peer and family support models for providing on-going self-management support for adults living with chronic conditions such as diabetes
  2. Understand gaps in the evidence and current weakness of current approaches
  3. Examine approaches that can incorporated into ongoing clinical care and highlighted best practices
