Objectives

After participating in this educational activity, attendees should be able to:
1. Discuss a case of a missed or delayed diagnosis resulting in patient morbidity or mortality
2. Identify systems-related causes of the error(s) in the case
3. Identify cognitive causes of the erorr(s) in the case
4. Discuss methods of preventing similar erorrs in the future
