Objectives

After participating in this educational activity, attendees should be able to:
1| Describe what a Community Health Needs Assessment is, including its role in identifying population health risks, key CHNA data sources, and why non-profit hospitals are required to complete one to support risk mitigation and community benefit planning.
2| Recognize the region's priority health needs and inequities identified in the 2025 CHNA., including populations at increased risk, health disparities, and areas of preventable adverse health outcomes.
3| Review how the CHNA report is used by Baystate to inform organizational strategy, resource allocation, and risk-informed decision-making, how it is used by community constituents, and how Baystate research staff may use the CHNA to inform evidence-based research and interventions.
