Goal:
The overall goal of the Geri-Palli TLC Immersion course is to foster collaboration among inter-professional teams in the management of complex older patients thereby providing thehighest standard of geriatrics and palliative care.

Target Audience
This course is designed for an inter-professional audience including but not limited to physicians, nurses, pharmacists, social workers, medical assistants, community health workers, administrators, case managers, clinical supervisors, medical directors, quality specialists, senior leaders, and those working in the community with older adults.
Learning Objectives
Age Friendly Framework (4Ms):
1. Identify & explain the 4Ms.
Dementia:
1. Recognize signs of dementia
2. Define the diagnosis criteria for dementia
3. Discuss dementia diagnosis with individual and family
Depression:
1. Identify atypical signs and symptoms of depression
2. Use depression screening tests such as Geriatric Depression Scale and PHQ-9; Describe treatment for depression
3. Describe treatment for depression
Drugs & Deprescribing:
1. Identify polypharmacy;
2. Memorize some common inappropriate medication from Beer’s List
3. Give examples of common drug to drug interaction
4. Perform appropriate deprescribing
Falls:
1. Perform screening for falls such as Get Up and Go Test
2. Describe ways to reduce rate of falls
Multimorbidities/Frailty:
1. Review local data
2. Describe AGS’s approach to care
2. Review Frailty tool such as Edmonton Frail scale
Community resources:
1. Discuss community resources for elderly such as Greater Springfield Senior Services, Alzheimer Association
2. Describe community activities such as day center, home help aids, meal on wheel
Prognostication:
1. Identify clinician limitation in prognostication
2. Perform ECOG and Palliative Performance scale
3. Identify signs and symptoms of poor prognosis
Digital Equity and Older Adults:
1. Describe the digital divide and it's impacts that cut across all aspects of life in particular for older adults
Serious Illness Conversation:
1. Describe tools to initiate serious illness conversation
2. Develop own phrase to initiate serious illness conversation
Palliative care and Hospice care:
1. Define palliative care and hospice care
2. Review hospice admission eligibility
Symptom Management:
1. Review common symptoms at end of life
2. Approach to pain, dyspnea and delirium
3. Describe concept of Total Pain
4. Discuss non pharmacological approach to pain
