Goal:

The overall goal of the Geri-Palli TLC Immersion course is to foster collaboration among inter-professional teams in the management of complex older patients thereby providing thehighest standard of geriatrics and palliative care.

Target Audience

This course is designed for an inter-professional audience including but not limited to physicians, nurses, pharmacists, social workers, medical assistants, community health workers, administrators, case managers, clinical supervisors, medical directors, quality specialists, senior leaders, and those working in the community with older adults.

Learning Objectives

Age Friendly Health Systems:
1.  Describe Age Friendly Health Systems
Quality Improvement 101:
1.  Identify current QI Projects at Baystate Health

PDSA & Practice Drill:
1.  Perform a practice PDSA cycle

Applying QI Tools:
1.  Recognize common quality improvement tools
TeleECHO & Project Planning:
1.  Explain the concept of force multiplier
2.  Identify small tests of change that can be made to improve care for older adults at your site
