Enhance attendee's skills in: recognizing and addressing common geriatrics and palliative care challenges such as dementia, falls, polypharmacy and prognostication; Identifying goals, discussing prognosis and guiding serious illness conversations and working in inter-professional teams to design quality improvement projects.

Target Audience
An interactive inter-professional team approach of mini-lectures, small group case discussions using the 4M's Age Friendly Framework designed for an interprofessional audience including but not limited to: physicians, nurses, pharmacists, social workers, medical assistants, community health workers, administrators, case managers, clinical supervisors, medical directors, quality specialists, senior leaders.
Learning Objectives
- Recognize and address common geriatrics and palliative care challenges such as dementia, falls, polypharmacy and prognostication
- Identify patient goals by discussing prognosis and guiding serious illness conversations
- List the core requirements for "age-friendly" health care for older adults

Your refund, less a cancellation fee, will be processed within two (2) weeks from the date we receive the written notification. For credit card refunds, please allow one to two (1-2) billing cycles for the credit to appear on your statement.
